Development of tailored home-based cardiac rehabilitation in frail patients


As a consequence of improved treatment and survival rates, frail cardiac patients with extensive comorbidity are common and will become more prevalent. These patients are at high risk of functional decline and hospital readmission. Home-based cardiac rehabilitation (CR) may prevent this. However, this has not been studied in frail patients. For these patients, I aim to: 1) identify modifiable determinants for physical functioning around hospitalization, 2) develop tailored home-based CR, 3) assess validity of a home-based exercise test (to replace a lab-based test), and 4) assess the effect of tailored home-based CR on physical functioning.
